About our practice
A family dental practice on Hazeldean Road in Kanata, led by Dr. Anahita Azadi since 2025.
Our story
Dr. Kyra de Gouw and Dr. Cynthia Simon opened Hazeldean Family Dental Centre in Kanata in 1997, the year they both graduated in dentistry from the Université de Montréal. Brigitte joined the next year as the practice's first office administrator, and several of today's team have been here for more than 20 years.
In 2025, Dr. Anahita Azadi took over the practice. She kept the team patients know, and both founding dentists continue to see patients here. Dr. Azadi provides dental implants, crowns, wisdom tooth extractions, gum treatment and gum grafting in the office alongside everyday family dentistry, so many patients can have an extraction, grafting, an implant and their new tooth all handled in one familiar place.
Care is available in English and French.
What you can expect from us
- Time to explain. We show you what we see, often with photos or x-rays, before we recommend anything.
- Options in writing. For larger treatment you get a written plan with costs, and time to decide.
- One office for the family. Check-ups for children, parents and grandparents in one place.
- Treatment under one roof. Implants and gum surgery are provided here, so you don't have to start over somewhere new.

Infection control
We follow the infection prevention and control standards of the Royal College of Dental Surgeons of Ontario. Instruments are cleaned and sterilized between patients, and single-use items are used once and discarded.
Getting here
We're at 457 Hazeldean Road, Unit 6, Kanata. Parking is free, and the office is wheelchair accessible. If you need any other help with your visit, tell us when you book and we'll plan for it.
